Consent to Services & Liability Release

By using the services of Dream Root Wellness, LLC, the Client is consenting to the services in full. The Client is acknowledging that Dream Root Wellness, LLC is not offering medical care or services under a medical license, and the Client should seek medical counsel prior to using any of the services provided by Dream Root Wellness, LLC.

The Client releases Dream Root Wellness, LLC from the liability of any loss or injury incurred by the Client while using the services of Dream Root Wellness, LLC.

Licensing & Medical Counsel

Dream Root Wellness, LLC promises that it holds all licenses necessary to perform the work, that such licenses are valid and effective as of the date any work is performed or services provided, and that all work performed or services provided will be done in compliance with all applicable federal, state, or local laws and regulations.

The Client acknowledges that the services provided by Dream Root Wellness, LLC neither constitute medical care or advice, nor are they performed as a licensed medical professional, and the Client should seek medical counsel prior to using any of the services provided by Dream Root Wellness, LLC.

Pre-Payment & Invoices

Dream Root Wellness, LLC requires pre-payment prior to booking of services unless otherwise noted. Dream Root Wellness, LLC may, when appropriate, invoice the Client after completion of services for required additional payments or charges unless otherwise noted in the Payment schedule. The Client agrees to pay invoice(s) by the due date(s) specified. Unpaid or overdue invoices may result in suspension or termination of services to the Client.

Payment methods

Payment will be made to Dream Root Wellness, LLC via cash, check, an approved payment card, or by any other payment method determined by Dream Root Wellness, LLC.

Cancellations & Reschedules

Dream Root Wellness, LLC asks that the Client cancel or reschedule at least 1 day in advance of any scheduled service.

The Client is responsible for the appropriate scheduling of services and the provision of valid contact information.

Dream Root Wellness, LLC will not provide refunds for canceled services.

Dream Root Wellness, LLC may suspend or terminate services to the Client for failure to pay outstanding charges.

Dream Root Wellness, LLC reserves the right to cancel or reschedule any service without explanation or notice. In these cases, Dream Root Wellness, LLC may provide a refund to the Client as appropriate.

Privacy

Dream Root Wellness, LLC will exercise all due diligence to protect the privacy of the Client. At no point does Dream Root Wellness, LLC sell or share the information of the Client, except when required by law or in cases where it is required for reimbursement from business partners (as when the Client clicks on affiliate links on the website, forum, blog, or podcast of Dream Root Wellness, LLC).
